what are exosomes?

Exosomes are nanoscale extracellular vesicles that facilitate cell to cell communication. They carry proteins, lipids, nucleic acids. They are signaling pathways that affect cell behavior. The rationale for using exosomes is that many of the exosomes are derived from mesenchymal stem cells. These stem cells can deliver regenerative signals.

What is the secretome?

The secretome is a family of your body’s secretions that are mobilized in response to stress or injury. Exosomes are one member of the secretome family. The secretome includes growth factors and other building blocks to repair tissue. (At one talk I went to last year by a bench researcher, he described exosomes as lumber to build the house. The secretome includes the other factors like nails, sheetrock, etc to build the house.) They used a great example that more exosomes is not always needed. Once you have enough lumber, you need the other factors to build a house.

How have they been used in aesthetic medicine?

Exosomes are being used in aesthetics to address multiple issues of aging at a cellular level. The science is looking at the details- delaying fibroblast senescence, normalizing extracellular matrix production, reducing inflammation, activating stem cell populations in your body.  In English what does this mean? What is the future of exosomes?

“Exosome” is a SUPER broad term. Please see my other blogs on exosomes (EXOSOMES BLOGS). What is the source (plant/human)? What is the concentration? What is the age of the exosome (Age Zero/older)? How is it processed and stored? There is need for more standardization, quality control, and studies to figure out how best to harness the real potential of these exosomes. Right now it is a bit of a wild west with what they claim they can do and standards for production/processing/etc.

Specific exosomes will likely come for specific tasks. Exosomes can come from adipose tissue (fat) “Adipose derived stem cell” ADSC exosomes. They can come from Dermal fibroblasts (HDF-Exos). Bone marrow derived mesenchymal stem cells. Dermal papilla cell exosomes for hair follicles. Perinatal derived exosomes, especially umbilical cord exosomes, are the “age zero” with high wound healing mediators, rapid skin penetration, adn reduced immunogenicity- so these may be the best rejuvenators.

I expect exosomes, growth factors, and stem cells will be a cornerstone of aesthetic treatments in the future. Regenerative medicine, natural results, minimal downtime. The future is coming. My goal is to make sure I am using the best science and best products for my patients.

How does Dr. Greenberg use exosomes currently?

I’m a fan. We use Age Zero exosomes only from two very reputable companies with good science behind them.

I think this is part of our future. With time I think we will be able to hone in how to use it better, duration, strength, type of exosome and more. Right now, we are using pluripotential embryonic exosomes, which seem to have broad capabilities.
